Cancellation & No-Show Policy
Any cancellation or reschedule made less than 24 hours of the appointment time will result in a cancellation fee of the full cost of service. The same policy applies to no-show appointments.
Please DO NOT cancel or reschedule via appointment reminder texts. These texts are no-reply messages. Your reply text will not be received and will not count as cancelling or rescheduling within 24 hours.

Please note, Rebalance Bodywork is not able to accept Premera BCBS plans at this time.
Massage benefits verification and letter of medical necessity (or referral) are required before the time of the appointment or full payment by the patient will be expected at time of service.
BCBS requires massage therapists to have a letter of medical necessity on file to validate the use of insurance benefits for massage services.
Some plans require an actual prescription, all other plans require a letter of medical necessity (LMN). A LMN is a doctor's explanation as to why massage services are needed for the patient. Regardless of what your plan language might state, BCBS's policy with massage therapists requires a LMN on file for all patients as massage therapists are non-diagnosing providers. A Medical Doctor, Doctor of Osteopathy, Naturopath, or Doctor of Chiropractic can provide a LMN, and are able to fulfill this request from patients via phone or a MyChart message. An in-person office visit is typically not required by doctors for an LMN request.
LMNs can be sent directly to Rebalance Bodywork via fax (360-519-5060), email, or brought in by the patient at their first appointment.
To determine if your plan has massage therapy benefits, complete the following steps:
1) Call the customer service number and choose the option for benefits and coverage.
2) Ask if your plan covers massage therapy.
3) Ask if massage therapy is covered as a stand-alone service, or if it has to be part of a PT or chiropractic treatment.
4) Verify that Christina Gagelmann is in your network - NPI#1538545249.
5) Ask how many sessions a year are covered.
6) Ask if the deductible applies and for the coinsurance/copay amount.
7) Make sure to get a reference number for your call.
